    That the Board approve the service agreement between Mansfield City Schools and Mid-Ohio ESC to provide Play-Based Physical Therapy services for the 2021-2022 school year.


    8.3 Agreement for Service - Highland Local Schools (attachment)

    That the Board approve the Service Agreement between Highland Local Schools and Mid-Ohio ESC for Erin Snyder, Speech Language Therapist, and Beth Gildenmeister, Occupational Therapist, to provide supplemental Occupational and Speech Language Therapy services to Highland Local Schools students on Home Instruction for the 2021-2022 school year.

    That the Board approve the agreement between Highland Local School District and Mid-Ohio ESC for Teaching & Learning services for the 2021-2022 school year, as outlined in the attached service agreement. 


    8.5 Agreement for Service - Hillsdale Local Schools (attachment)

    That the Board approve the agreement between Hillsdale Local Schools and Mid-Ohio Educational Service Center for Initial CPI training to be provided by Angela Fetter on October 15 and November 29, 2021.


    8.6 Agreement for Service - Plymouth-Shiloh Schools (attachment)

    That a service agreement contract be approved between Mid-Ohio Educational Service Center and Plymouth Shiloh School District for Angela Fetter, BCaBA, to provide behavior consultation services, beginning October 1, 2021, and ending June 30, 2022.

    That the Board approve the agreement between Heartland Academy and Mid-Ohio ESC to provide Resident Educator Mentoring services for the 2021-2022 school year.

    That the Board approve the agreement for service between Crestline Exempted Village Schools, St Joseph School-Crestline and Mid-Ohio ESC to provide Physical Therapy services on an as-needed basis during the 2021-2022 school year.

    That the Board approve the agreement between Crestline Exempted Village Schools and Mid-Ohio ESC for School Psychologist services provided by Susan Taylor, beyond the .6 FTE for the 2021-2022 school year. 


    8.11 Agreement for Service - Willard City Schools & St. Francis Xavier School (attachment)

    That the Board approve the agreement between Willard City Schools, St. Francis Xavier School and Mid-Ohio ESC to provide a presentation to school staff on October 22, 2021. 


    8.12 Agreement for Service - Mt. Gilead Exempted Village Schools (attachment)

    That the Board approve the agreement between Mt. Gilead Exempted Village Schools and Mid-Ohio ESC to provide vision services for the 2021-2022 school year. 


    8.13 Agreement Appointing Business Advisory Council (BAC) - Mt. Gilead EVSD (attachment)

    That the Board approve Mid-Ohio Educational Service Center's Business Advisory Council (BAC) to represent Mt. Gilead Exempted Village School District. 


    8.14 Agreement for Service - North Central Ohio ESC & St. Mary of the Snow School (attachment)

    That the Board approve the agreement for service between North Central Ohio ESC, St. Mary of the Snow School and Mid-Ohio ESC to provide Learning Loss Assistance (Tiffany Stout) funded by EANS funds, on an as-needed basis during the 2021-2022 school year.


    8.15 Authorize Attendance - SAS Training

    That the Board approve an out of state travel for Kelly Britton to attend the SAS training in November, 2021 to be held in North Carolina. 


    8.16 RESCIND - Agreement for Services - Pioneer CTC

    That the Board rescind the agreement between Pioneer Career and Technology Center and Mid-Ohio ESC for Cathy Csanyi to provide In-District PBIS, Positive Behavior Intervention and Support Training on October 12 &13, 2021.


    8.17 Ohio Department of Education Grants

    That the Board accept the following Ohio Department of Education Grants for FY22:

    Crawford Partnership for Education and Economic Development -$30/hr, not to exceed $15,000, to provide FAFSA Funding Opportunities Grant/College Coordination services, provided by Sherri Tinch-Greter.

    Lisa Kuhar - $50/hr. plus mileage and lodging to provide Navigating Resident Educator Mentor training at Mid-Ohio ESC, as authorized by the Director. 

    Mansfield City Schools - $100/hr. (not to exceed $4,000 total) to provide BCBA supervision for Angela Fetter, for RBT Summer Training, as authorized by the Director 

    Dr. Staci (Dahni) Reynolds - hourly per contract to provide BCBA supervision, as authorized by the Director. 

    Shelby City Schools - $3,000 (estimate), to provide Fiscal Re-Design/SCView Consulting and Training, as authorized by the Treasurer. 

    Linda Williams - $50/hr. plus mileage and lodging to provide Navigating Resident Educator Mentor training at Mid-Ohio ESC, as authorized by the Director. 


    8.21 Governing Board Resolution to Establish Educational Requirements for Substitute Teachers for the 2021-2022 School Year. (available for review)

    That the Board establishes flexible educational requirements for substitute teachers, per S.B. 1, for the 2021-2022 school year. 


    8.22 Updated Substitute/Tutor List 2021-2022 (available for review)

    That the Board approve the updated Substitute Teacher/tutor List for the 2021-2022 school year.


    8.23 Agreement for Service - Galion City Schools (attachment)

    That the Board approve the agreement between Galion City Schools and Mid-Ohio Educational Service Center for training and consulting services in support of the Comprehensive Literacy State Development (CLSD) Grant, provided by the Teaching & Learning team, for the 2021-2022 school year.

    Amanda Braen, per diem - Physical Therapist Assistant services on an as needed basis (Crestline / Crestline St, Joseph)

    Sherri Richter, $1,000.00 ($500.00 per teacher) to provide Resident Educator Mentor services for (2) two teachers (Heartland Academy)
