    7. Superintendent Recommendations - Operational Action

    7.1 Agreement for Service - Crestline Exempted Village School District (attachment)

    That the Board approve the agreement with Crestline Exempted Village School District and Mid-Ohio ESC to provide Initial Crisis Prevention and Intervention Training on January 5 and 6, 2021.


    7.2 Agreement for Service - Crestline Exempted Village School & St. Joseph School, Crestline (attachment)

    That the Board approve the agreement for service between Crestline Exempted Village Schools, St Joseph School-Crestline and Mid-Ohio ESC to provide Speech and Language and Occupational services on an as-needed basis during the 2020-2021 school year.


    7.3 Agreement for Service - Galion City Schools (attachment)

    That the board approve the  agreement for service between Galion City Schools and Mid-Ohio ESC to provide audiology services for the 2020-2021 school year.


    7.4 Agreement for Service - Mansfield City Schools (attachment)

    That the Board approve the service agreement between Mansfield City Schools and Mid-Ohio ESC to provide Play-Based Physical Therapy services for the 2020-2021 school year.


    7.5 Agreement for Service - Mansfield City Schools (attachment)

    That the Board approve the agreement between Mansfield City Schools and Mid-Ohio ESC to provide Occupational Therapy Assistant services for the 2020-2021 school year.


    7.6 Agreement for Service - Cleveland State University (attachment)

    That the Board approve the Affiliation Agreement between Cleveland State University School Psychology Program and Mid-Ohio ESC.


    7.7 REVISED Agreement for Service - School Psychologist Candidate in the "Grow Your Own Program" (attachment)

    That the Board approve the REVISED agreement with Claire Brooks, School Psychologist candidate in the "Grow Your Own Program" with the University of Toledo.


    7.8 Governing Board Resolution to Establish Educational Requirements for Substitute Teachers for the 2020-2021 School Year.

    7.9 Purchased Service Agreements (attachment)

    EJ Therapy - $80/hr  to provide audiology services on an as-needed basis for the remainder of the 2020-2021 school year.

    Zack Dollish - $100, photographer for the Franklin B. Walter Program on March 16, 2021.

    Sandra Donah - $100/video x 3 = $300 to create three (3)  morphology videos for Elementary, Middle School and High School as professional development for teachers, to be paid through the Striving Readers Grant funds.


    7.10 Updated Substitute/Tutor List 2020-2021 (available for review)

    That the Board approve the updated Substitute Teacher/tutor List for the 2020-2021 school year.


    8. Superintendent Recommendations - Personnel Action

    8.1 Employment Contracts

    That the following personnel contract be approved effective with the 2020-2021 contract year (*contingent upon client service agreement, administrative agent agreements, and/or receipt of grant funds and proper certification/licensure and completed background checks).

    Crystal Bischoff    Occupational Therapist Assistant    from 80 days to 122 days

    Joanna Greenwalt-School Psychologist, $414.63/daily (7.5 hr day/wk), not to exceed 18 days,  January 11, 2021 through June 1, 2021

    Sheryl Whitley - Administrative Assistant, $15.70/hr additional workdays, as-needed 


    8.4 Resignation

    Kathleen Grove, Administrative Assistant, effective at the end of the work day on January 14, 2021.
